Step By Step Transitional Program
When recovering from an illness, injury or surgery, The Orchards at Bartley provides the services required to prepare patients for the next step in their journey to wellness.
Program Criteria
To participate, patients must have one of the following needs:
- Assistance with transferring - may require hands-on assistance with getting from the bed to the wheelchair or the wheelchair to a dining chair, etc.
- Assistance with transferring - may require reminders or preparation
- Assistance with meal preparation - all meals are provided
- Assistance with bathing/dressing - may require assistance in the shower or assistance with dressing and undressing
Services Included in Daily Rate:
- Fully furnished room (bed, dresser, night stand, recliner, linens, towels)
- 24-hour, on-site supervision by highly trained staff
- Medication management
- Assistance with bathing and dressing
- Three chef-prepared meals served in our formal dining room
- Nursing supervision
- Daily activities
- Laundry (done weekly)
- Housekeeping services
- Dietician services
- Transportation to medical appointments
- Transportation to outpatient therapy
- 24-hour call bell emergency system
- 24-hour nursing available*
- Equipment rental*
- On-campus physical, occupational and speech therapy*
*Additional charges may apply. Medicare and other insurance may cover some or all of your additional needs.
The Admission Process

- Patient's physician or social worker can contact The Orchards to make referral
- Our nursing staff will visit with the patient to discuss his or her needs
- We will develop an individualized program and plan of care for each patient
- Patient is on the way to recovery
Length of Stay
Length of stay can be as little as two weeks or as long as two months. Our program is designed to meet patients' needs for as long as they need the extra helping hands.
Rates
Daily rates are based upon individual needs. We will review all program criteria and costs with patient during our initial meeting.
